Sub-acute mental health support is designed for individuals who need more intensive care than regular community services but less than what a hospital provides. We offer three specialised programs tailored to support you during this critical phase:

Transition to Recovery (TRec)

Intensive community outreach to help individuals transition from hospitalisation.

Southside Step Up Step Down (SC SUSD)

Residential and outreach support for those becoming acutely unwell or in early recovery from acute mental illness.

The Way Back Support Service

A voluntary, non-clinical suicide aftercare program designed to provide support and guidance to for those who have experienced a recent suicide attempt.
