The Transition to Recovery program (TRec) is an intensive community outreach mental health program to assist people to transition to the community following an acute episode of mental illness and to prevent relapse.
The program is designed to help individuals maximize their potential for recovery and independence after being discharged from the hospital or for those who require additional support in the community to manage sub-acute symptoms and prevent hospitalisation.
This program runs for up to 12 weeks and offers intensive outreach support, including calls on evenings and weekends.
Here is what the program can provide:
If you are currently at the Adult Mental Health Unit at Canberra Hospital or the Acacia Adult Mental Health Unit at North Canberra Hospital, TRec may be able to assist you. Additionally, if you are connected with an ACT mental health team and are at home but feel you need extra support to prevent hospitalization, we can also help you through this program.
Individuals are eligible and can voluntarily access the program if they meet the following criteria:
TRec can also assist eligible individuals who are homeless.
Referrals to the program are made by clinicians from one of ACT Health’s mental health teams, which include:
Carers often recognise the early warning signs of a mental health episode in their loved ones. The TRec program is most effective when individuals seek assistance as early as possible. If you are worried about a family member, please discuss early intervention options with TRec.
