About the Program
The Transition to Recovery (TRec) program is a short-term, intensive community outreach service that supports people following an acute episode of mental illness.
We work alongside you to build confidence, strengthen independence, and support your recovery journey in the community.
This Program is an ACT Government funded initiative.

What the Program Includes
Outreach support Monday to Friday, 9am–5pm
Evening phone support until 9pm, including weekends
Help linking with services and supports
Assistance attending appointments and community programs
A safe space to talk about mental health and daily challenges
Psychosocial education to support recovery and wellbeing
Up to 12 weeks of intensive support.

A TRec Worker supports you to manage your mental health and daily life after discharge from hospital, or when you need extra support to prevent hospitalisation.

Manage ongoing mental health symptoms
Reconnect with supports and services
